Proof of Demise was an achievement awarded by completing the Northrend daily heroic instance quests. Only one of these quests are available per day, at random, from Archmage Lan'dalock outside of the Violet Hold in Dalaran. It would take a minimum of 12 days to complete, one for each quest.
